ST. MARTIN DE PORRES LECTOR MINISTRY COVENANT & PARTICIPATION FORM

We are now beginning the yearly process for renewing and recruiting Liturgical Ministers. A current list of those actively engaged in ministry is compiled for the Parish Liturgy Dept. We ask you to prayerfully discern your commitment as a Lector by completing this Covenant Form and returning it by July 31st. You can submit this form “on-line” (click “submit” which sends the form directly to liturgyscheduler@smdpyl.org) or send a printed copy via FAX (714-970-5654) or deliver to the Parish Office, Attention: Roz/Liturgy Dept. Thank you for your participation in the liturgical life of St. Martin’s!

Note: If we do not receive this Covenant form from you, we will assume you no longer wish to remain active at this time. If at any time you desire to be put back on the active schedule, please contact Roz/Liturgy Dept. @ 714-970-2771.
