Continuing Faith Formation Grades 3-5: Copy of Baptism certificate and 1st Eucharist certificate, photo of student
Confirmation: Copy of Baptism certificate and 1st Eucharist certificate, photo of student
Adult Confirmation: Copy of Baptism certificate and 1st Eucharist certificate
RCIA: No certificates or Catholic sacraments needed
